Summary: Will be responsible for ensuring a care plan is in place that helps the patient with clinically significant illness reach their health goals. Also responsible for all vaccine management activities, including training of new staff and assisting in the development of community education.

Essential Duties And RESPONSIBILITIES OF patient NAVIGATOR:

  • Work with the healthcare team to identify any patient with a new significant health diagnosis.
  • Act as a liaison between the service providers and advocate for patient in the establishment of a treatment plan that meets their medical, social and cultural needs.
  • Regularly review the patient’s progress toward goals.
  • Identify and determine eligibility of patient for supportive services and resources within the community, and assist them in the application process. Contact medical specialty offices to stay informed of supportive services they may offer.
  • Identifying barriers to care experienced by the patient, their families and caregivers in order to guide them through the complexities of the healthcare systems.
  • Assist patients with appointment information (i.e. directions to the appointment, handicapped accessibility, language barriers) to health specialists.
  • Transport and accompany patients to medical appointments if needed.
  • Researches the medical diagnosis using evidence-based sources and conveys complex information to the patient and family in a manner that is easily understood.
  • Provide follow-up care and pre-planning for the next appointment.
  • Explain the function of the health care team members who will implement the plan of treatment to the patient.
  • Work with hospitals on discharge plans for patients.
  • Coordinates with Community Health representatives to provide education in both individual and group sessions to promote patient understanding of various medical conditions.
  • Quantify and track patient care activities.
  • Accountable for vaccines administered through the timely data entry of patient demographics and inventory to the NYSIIS system.
  • Work with various departments within the medical facility to implement policies and procedures to guide the daily operations of receiving, storing, ordering and returning vaccine.
  • Install calibrated continuous temperature monitoring devices, record daily temperatures, review weekly to identify out of range temps.
  • perform physical invetory of all vaccine stock, rotate first to expire.
  • Performs as Staff Nurse when needed
  • Other duties as assigned

supervisory Responsibilities: There are no supervisor requirements for this position.

  • Complete Patient Navigator Training within one year of hire date. Participate in continuing professional development programs as required.
  • Complete mandatory NYS vaccine training modules.

Education And/or Experience:

  • Registered Nurse from accredited nursing program in the United States, NYS licensure AND
  • Two years related experience.
  • One-year experience in chronic disease case management preferred.

Other Skills and Qualifications: Knowledge of Mohawk language and traditions is an asset. Skilled in the use of computers and related software applications, and accessing internet resources.

Reasoning Ability: Ability to provide counseling and emotional support with compassion and respect for the individual/family.

Must have and maintain a valid driver’s license in accordance with the Saint Regis Mohawk Tribe’s Fleet Policy.

The successful candidate must pass a test for illegal substances and background check prior to employment being confirmed.
