Summary: Under the supervision of the Home Health Aide Supervisor, the Home Health Aide is responsible for providing various tasks such as personal care, housekeeping, range of motion exercises, medication prompts, meal preparation, and companionship to patients in their homes in accordance with an established care plan.   

 

Essential Duties And Responsibilities: 

  • Follow all policies and procedures within Chronic Care and SRMHS. 
  • Successfully complete a basic training program in home health aide services or an equivalent exam approved by Chronic Care and possess written evidence of such completion. 
  • Participate in training, workshops, and annual skills competency testing, pertinent to the scope of practice 
  • Document care provided to the patient, such as the date and time of care, specific tasks performed, patient condition, and progress, and discuss any abnormal observations with the supervisor or nurse case manager. 
  • Assist patient with transfers using proper body mechanics and assistive devices such as gait belt, transfer board, or mechanical lifts to reduce the risk of strain or injury. 
  • Prompt patients to take prescribed medications under the written direction of a physician or as directed by the case managing nurse. 
  • Provide personal care to patients with activities of daily living such as bathing, grooming, toileting, and dressing, providing hygiene care as needed to keep the patient’s skin clean and dry. 
  • Provide housekeeping including but not limited to decluttering, changing bed linens, washing and folding laundry, kitchen cleaning, sweeping, mopping, vacuuming, bathroom cleaning, and taking trash out. 
  • Provide companionship, entertain, and converse with clients to keep them mentally healthy and alert. 
  • To provide a safe and caring environment that promotes patients’ physical, emotional, and psychological well-being. 
  • Apply assistive devices as instructed by the case managing nurse to prevent skin breakdown over pressure points. 
  • Respecting patient privacy and dignity is of utmost importance, and home health aides must maintain confidentiality and protect the personal information of their patients. This includes not discussing personal details or medical information with unauthorized individuals. 
  • Maintaining professional boundaries is essential for home health aides to ensure a professional and appropriate relationship with their patients. 
  • To accept other duties as assigned. 

 

Supervisory Responsibilities: There are no supervisor requirements for this position.

Education And/or Experience: 

  • High School Diploma or GED 

 

Preferred - Personal Support Worker Certification or equivalent  

 

Other Skills and Abilities:  Communication both orally and written, Mohawk language and traditions are preferred, physically able to carry out the duties of the position, and maintain confidentiality.
