Summary: Under the supervision of the home health care supervisor, the home health aide is responsible for providing personal care, housekeeping, range of motion exercise and companionship to patients in their homes in accordance with an established care plan.
Essential Duties And Responsibilities:
- Follow all policies and procedures within Outreach Chronic Care and SRMHS.
- Home health aides shall have successfully completed a basic training program in home health aide services or an equivalent exam approved by the Outreach Department and possess written evidence of such completion.
- Participate in training, workshops, and annual skills competency testing pertinent to the scope of practice
- Maintain records of client care, condition, progress, or problems.
- Report and discuss observations with the supervisor or nurse case manager.
- Assist patient with transfers with gait belt and transfer board if needed.
- Assist prescribed oral medications, under the written direction of a physician or as directed by the case managing nurse, and ensure patients take their medicine.
- The home health aide will provide personal care, housekeeping, ROM (Range of Motion) exercises, assist with medications, and, most importantly, provide companionship to the elderly and medically ill patients.
- Provide housekeeping, including but not limited to changing bed linens, washing and ironing laundry, and cleaning.
- To provide care throughout the day, checking patient at regular intervals according to their care plan.
- Provide companionship, entertain, and converse with clients to keep them mentally healthy and alert.
- Provide hygiene care to keep the patient’s skin clean and dry.
- To respond promptly to calls for assistance while respecting, at all times, the patient's privacy, dignity, and choice.
- To provide a safe and caring environment that promotes patients’ physical, emotional, and psychological well-being.
- Follow the turning and repositioning schedule.
- Apply assistive devices as necessary to avoid skin breakdown over pressure points.
- To accept other duties as assigned.
Supervisory Responsibilities: There are no supervisor requirements for this position.

Education And/or Experience:
- High School Diploma or GED, OR
- Personal Support Worker Certification or equivalent preferred
Other Skills and Abilities: Communication both orally and written, Mohawk language and traditions are preferred, physically able to carry out the duties of the position, and maintain confidentiality. Should be able to lift more than 50 pounds and stand for extensive periods of time.
