Summary: As a Primary Care Family Nurse Practitioner you will provide direct patient care, including diagnosis, treatment plans and dispositions; and play a key role in driving improvements in primary care, quality initiatives, and patient satisfaction, while aligning with program goals and corporate values.

 

Essential Duties and Responsibilities include the following. Other duties may be assigned.

· Patient-centered nurse practitioner caring for the Akwesasne community.

· Provider primary care to patients across the age spectrum from newborn to geriatric.

· Orders prescription medications and diagnostic procedures.

· Authorizes and coordinates all specialty and ancillary services.

· Generally, have 16 encounters daily, provider will be flexible to accommodate patient and team needs, and may occasionally see more patients.

· Will participate in a Saturday morning rotation with other staff providers.

· Focus on team-based, patient-centered care.

§ Will work closely with RNs, LPN’s, CHR’s, front desk staff, and administrative support staff in addition to provider colleagues.

§ Will also collaborate with other patient support services across the organization.

· Correctly and completely documents entries and maintains patient charts in a timely manner as outlined in the Medical Clinic policies and procedures.

· Interacts with all colleagues and learners in supportive, productive manner.

· Remains current in the field of medicine and practices up to date medicine as established by national standards (ACOG, AAFP, CDC, etc), with practical application based on patient preferences and clinical environment.

· Will be a steward of healthcare resources by following protocols for patient specialty consultation and referrals within and outside the SRMHS. Works closely with the Business Office program and care team referral navigators as needed to ensure patient has access to most appropriate care. Accurately and promptly provides coding, billing or other financial information to SRMHS billing team and follows up insurance requests as required.

· Represents SRMHS in a positive and respectful manner in all circumstances including with staff, community members, colleagues, and professional organizations. Coordinates and participates in community outreach programs, serves on SRMHS committees, serves on hospital committees, fully engages in the health of the Akwesasne community.

· Will often manage multiple tasks simultaneously, including telephone, computer operations and in-person visits while maintaining thoughtful communication with team members and patients.

 

SUPERVISORY RESPONSIBILITIES: This position has no supervisory duties

 

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Basic Life Support (BLS) must be obtained within 45 days of employment.

 

EDUCATION AND EXPERIENCE:

· Graduate of an accredited Nurse Practitioner Program and current unrestricted licensure by NYS as Nurse Practitioner; WITH

· Certification in family health; WITH

· Two (2) years of advanced clinical practice experience as a licensed Nurse Practitioner and current unrestricted licensure by NYS as Nurse Practitioner.
