SUMMARY: Under the direct supervision of the Program Manager, the employee will work with the NYS Women, Infant, & Children Program and Saint Regis Mohawk Health Services Health Promotion & Planning Program to assess the health needs of individuals and communities.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Plan and deliver education activities for various age groups consisting of informational displays, group discussion, newsletter development, community events, physical activity instruction, and cooking clubs,
  • Lead efforts to develop and administer surveys for identifying health concerns in the community and to develop programs that meet those needs. For example, they may help to organize blood-pressure screenings or classes on the proper installation of car seats.
  • Develop public health campaigns on a regular basis using monthly health observances
  • Provide basic health assessments such as first aid and height and weight measurements
  • Provide informal counseling and social support
  • Create workplace programs or suggest modifications that focus on wellness. For example, they may develop incentives for employees to adopt healthy behaviors, such as controlling cholesterol, or recommend changes in the workplace to improve employee health
  • Complete annual Medicare wellness visits with patients under the direction of their provider
  • Complete all documentation and reports required in a timely manner, and meet all deadlines that are assigned,
  • Inspect and document food code compliance for Tribal program kitchens and congregate areas as needed,
  • Will serve as Safety and Fire office for Health Services facility by conducting quarterly fire drills and monitoring compliance related to safety in the building
  • Will work with Program Manager to develop PSAs, Live cooking demonstrations, and video productions with providers on health topics
  • Ensures WIC benefits are tailored to meet participant needs/preferences and issued in accordance with state and federal policies.
  • Schedule and update WIC participant demographic when needed,
  • Ensures effectiveness and efficiency in the delivery of services.
  • Other duties as assigned.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  • Able to provide friendly customer service and work various schedules including evenings and weekends
  • Ability to lift up to 50 pounds
  • Must have excellent computer skills in excel, power point, word and flyer development
  • Excellent organizational skills are a must as you must be able to multi-task
  • Willingness to continue education through course work, WIC training, Health Coach certification, CHR certification, and food handler certification

EDUCATION and/or EXPERIENCE:

  • Bachelor’s Degree in Nutrition, AND
  • Two 2 years’ experience public health, community nutrition or related field

OR

  • Associate’s in Dietetics Technician, AND
  • Two 2 years’ experience public health or community health

OR

  • Bachelor’s degree in an appropriate health-related field from an accredited college or university such as Public Health or Community Health, AND
  • five (5) year’s work experience Public Health, Community based prevention programs or related field preferred.

REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
