SUMMARY: Under the direction of the Outpatient Program Coordinator, the Case Manager will assist the A/CDP Outpatient staff with supporting clients needs by helping to expedite and coordinate access to treatment and recovery services, assist with scheduling, collecting and managing data. This position will also serve as the Peer Advocate by providing support, education and advocacy to clients and linking them to resources in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES OF A CASE MANAGER:
- Work as part of the A/CDP team to provide quality client care.
- Work with counselors to ensure quality case management.
- Coordinating client care by helping counselors gather and process referrals to other agencies for inpatient treatment, detoxes or halfway houses.
- Ensure the smooth transition of referrals necessary to help clients with their treatment needs according to their treatment plan.
- Use the RPMS scheduling program to schedule clients when needed and extract data for reporting.
- Document clients contact in the Behavior Health System (electronic health record) according to New York State Office of Alcohol Abuse Services (OASAS) requirements.
- Maintaining accurate client records in accordance with NYS OASAS and IHS to write summaries, monthly and yearly reports, letters and memos.
- Assist in maintaining counselor caseloads, group lists, active client roster, and organizing the digitalizing process for client files.
- Maintain harm reduction inventory (Narcan, fentanyl and xylazine test strips) and provide training to other programs and community members.
- Provides administrative and receptionist duties when needed.
- Promotes the Saint Regis Mohawk Tribe in a positive manner.
- Accept other duties as programming needs develop.
ESSENTIAL DUTIES AND RESPONSIBILITIES OF A PEER ADVOCATE:
- Provide non-clinical crisis support when needed.
- Provide transportation to and from on-site appointments for clients when needed.
- Provide advocacy, outreach and engagement of clients and their families in forming a supportive, recovery network.
- Help clients and families to overcome barriers, ambivalence, and challenges in services.
- Provide transitional support and self-help tools to assist individuals as they move toward care and recovery.
- Partner effectively with other programs and staff to insure effective connections.
- Network with other agencies, groups, and organizations within the addictions and human services fields.
- Link Clients to resources in the community.
SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position.
QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and /or ability required. Ability to handle delicate role issues and to respond to crisis. Due to the sensitive nature of information, must maintain strict confidentiality at all times. Excellent oral, written communication skills and computer sills are required.
- Familiarity with Akwesasne Community an asset.
- Must have a thorough understanding of the disease concept of addiction and how the disease affects the family and Native American communities in general.
- Must have two years of sobriety (if a person in recovery). Must not abuse alcohol and/or any other drug for a non-recovering person.
- Must be obtain within three (3) years of employment the Certified Recovery Peer Advocate (CRPA) certification.
EDUCATION/EXPERIENCE:
- Associate’s Degree in a related field is the minimum requirement for the position.
OR
- Two (2) years work experience and/or training in case management or administrative work
- Two (2) years work experience in alcoholism/chemical dependency field or related area is PREFERRED.
REASONING ABILITY: The ability to communicate effectively verbally and in writing as appropriate for the audience so others will understand. Ability to solve practical problems and deal with a variety of daily situations.
